GENERAL PURPOSE
The Central Medication Management Clinical Pharmacist works with the Central Clinical Medication Management Team and collaborates with other stakeholders to provide individualized Medication Therapy Management Services to patients nationwide. The clinical pharmacist will identify, resolve and prevent medication related problems and assess each of the patient’s medications for indication, effectiveness, safety and adherence issues and incorporates lab data in the assessment process. The clinical pharmacist prepares a medication summary and patient centric care plan with recommendations as needed. The clinical pharmacist will collaborate and communicate with all members of the Consana team including prescribers and follows up as required to monitor the patient’s drug therapy and to ensure positive outcomes.

Primary responsibilities Include
ESSENTIAL JOB FUNCTIONS
- Evaluates medication profiles to determine if changes are warranted to optimize the clients drug therapy and minimize the potential for adverse drug reactions
- Utilizes and evaluates appropriate drug references, clinical literature, and current guidelines
- Evaluates collected patient data including vitals, labs, alerts, and pharmacogenomic reports to ensure appropriate medication selection and dosage; Performs pharmacokinetic calculations when necessary
- Collaborates with CMMP and provides recommended optimal medications, alternative dosing, or other modifications (including discontinuation of medications) as appropriate
- Prepares and communicates therapeutic plan recommendations with remote CMM Pharmacist
- Utilizes information collected by CMMP during interactive, person-to-person, interviews with patients conducted either in-person or via telephone or video conference
- Strives to provide excellence in pharmaceutical care and improve patient outcomes
- Communicates with all stakeholders to avoid communication gaps in patient care; provides monitoring and follow up information
- Maintains flexible availability to accommodate the needs of our patients and our business
- Documents clinical services according to established policy and format
- Performs MTM related services, including documentation and follow-up, within established time frames
- Maintains comprehensive knowledge of policies and procedures and performs in accordance with applicable regulatory and accreditation requirements or standards as they relate to assigned responsibilities and functions
- Ensures maintenance and compliance with clinical standards of the MTM services provided and remains up-to-date with clinical knowledge
- Communicates and articulates information in a clear, concise, and professional manner
- Demonstrates excellent listening skills and demonstration of empathy with patients in tone, language, and counseling
- Provides oversight and supervision to pharmacy technicians and pharmacy interns as required
WORKING CONDITIONS
- Interact with patients, caregivers, staff, clinicians, government agencies / personnel, etc. under all conditions / circumstances
- May be subject to hostile and emotionally upset patients, caregivers, staff, clinicians, etc., under all conditions / circumstances
- May be subject to travel as required
- May office out of Consana central location or work remotely as required
Required Qualifications
- Licensed Pharmacist in good standing with respective state board(s) of pharmacy; Preceptor registration preferred
- Pharmacy degree from an accredited U.S. School of Pharmacy, Pharm.D. preferred
- Completed Medication Therapy Management Services certificate program, or within 3 months post hire
- Ability to obtain NPI and other required credentials
- Have no restrictions on ability to participate in private, federal or state insurance programs including Medicare and Medicaid
- Basic computer skills
- Strong organization skills including the ability to prioritize and manage multiple tasks
- Strong clinical and analytical skills; ability to quickly identify problems and find effective solutions
- Strong written and oral communication skills; Ability to communicate with other team members working remotely
- Ability to make independent decisions
- Ability to effectively and harmoniously interact with patients and members of the care team at all levels;
- Promotes teamwork and professionalism
