Reviewing and monitoring prescription drug therapy is a part of your visit with your doctor. However, because your doctor has so much medical information to review and evaluate before and during your visit, they typically don’t have the time to go through each medication to ask and answer questions with you. They may not even know all of the medications you are taking if they have been prescribed by another doctor, or if you take over-the-counter medications, vitamins, or supplements.
Medicare may cover some medication therapy management services depending on your plan, how many chronic health conditions you have, how many covered medications you take and the amount of money you are projected to spend on those medications in the upcoming year. Unfortunately, even if you do qualify, many patients do not end up receiving these medication therapy management services. The way these traditional models work often relies on dispensing pharmacies that have little time and resources to spend on medication therapy management. There are several things you should have on hand to make the best use of your time with the clinical pharmacist. Luckily, these are all things you should have on hand or be able to get without much of a wait.

  • Every medication you are taking. Gather up every bottle, box, card, inhaler, over - the - counter product, vitamin and anything else you have around the house for review. If you are no longer taking anything you find this is a good opportunity to discard it to avoid confusion or accidents.
  • A list of your current medication treatments from the last time you went to the doctor, hospital, specialist, ER, rehab facility, etc.
  • The names and contact information for doctors and pharmacies you visit.
